醛固酮(Aldosterone)是一種由腎上腺皮質(zhì)中的膽固醇合成的類固醇激素。醛固酮在腎臟和肝臟中代謝,并作為控制鈉鉀平衡的關(guān)鍵鹽皮質(zhì)激素發(fā)揮作用。腎上腺合成和釋放醛固酮主要受腎素-血管緊張素-醛固酮系統(tǒng)(RAAS)的調(diào)節(jié),RAAS主要作用是調(diào)節(jié)和維持血壓、水電解質(zhì)平衡和維持人體內(nèi)環(huán)境穩(wěn)態(tài)。檢測血清醛固酮和血漿腎素在臨床上用于區(qū)分原發(fā)性和繼發(fā)性醛固酮增多癥。原發(fā)性醛固酮增多癥(醛固酮增多癥)的特征是腎素與醛固酮比率非常低,導(dǎo)致鈉滯留和血壓升高,通常是腎腺增生或腫瘤的結(jié)果。在繼發(fā)性醛固酮增多癥中,醛固酮的過量產(chǎn)生是由心力衰竭和腎動脈疾病等外部條件引起的。
